Thank You

We want to thank you for allowing our hospice team to provide grief support services to you for the past 13 months. In many ways it probably seems like it was just yesterday, but in other ways it may seem like it’s been forever since your person died. Remember, grief is a journey, not a destination. Our hope is that our grief support team has been a source of help and comfort to you during this difficult time.

We have now come to the end of our time with you. While your grief journey isn’t complete, you now have tools, tips and resources that will help you in the days ahead. Refer to those resources often for support.

We would like to encourage you to continue to join our online grief group when you can. You may benefit from sharing time with others who have also experienced grief.

Soon you will be receiving a survey in the mail from Strategic Healthcare Programs (SHP) concerning our grief support services. We would appreciate your feedback. Please complete the survey and return it to us in the mail. Your input will help us to continue to provide great support to those who need it in the future.

Thank you again for allowing us to walk alongside you during this time. We wish you all the best.

With You!
Jim McClure, Director of Grief Support Services

Volunteering

The Carnation Foundation

Celebrate the life of your loved one by creating lasting memorials in their honor through The Carnation Foundation. Our mission is to support patients and employees of Agape Care Group by providing financial assistance and funding programs that improve their quality of life. By partnering with us, you can make a meaningful impact in memory of your loved one. Whether you choose to donate in their name, sponsor a program that was close to their heart, or establish a memorial fund, your contribution will make a difference for those in need. Your generosity will not only honor the memory of your loved one, but also help support individuals who are facing difficult times. At The Carnation Foundation, we understand the importance of commemorating the lives of those we have lost. By working together, we can ensure that their legacy lives on through acts of kindness and compassion. Join us in making a difference and celebrating the life of your loved one through meaningful memorials that benefit others in their honor.
